Use this form to provide a medical history of your child if he/she wishes to play sports (Physical Form).

Self-Medication Release Forms
This form must be completed in addition to the routine district medication form for students who request permission to carry their own medication on campus or keep this medication in a P.E. locker.

Medication Authorization Form
Per NYS Medication Procedures, this form is necessary in order to administer ANY medication during the school year!
Please Note: This form must be filled out for both prescription and over-the-counter drugs.
